Insurance & Billing

Charges incurred at SurgeCenter of Louisville will consist of several components.  There will be a facility fee, a surgeons’ fee, an anesthesia fee for anesthesia cases, and a laboratory and/or pathology fee if applicable.  Each component is billed separately, so you will receive two or more bills for your surgery.  

As a courtesy, our Insurance Specialist - Wanda Kopple , will verify your benefit coverage with your insurance company.  She will contact you to discuss your estimated financial responsibility based on the information received from your insurance company.  This will include your deductible, co-pay, and/or co-insurance.  This information is estimated, based upon the procedure/procedures scheduled.  If additional or different procedures are necessary, it will affect your final bill. Any amounts not covered by your insurance (deductible, co-pay and/or co-insurance) will be collected on the day of surgery.   We accept the following forms of payment:  cash, personal checks, Visa, MasterCard, American Express and Discover.  In an effort to assist you with your financial responsibility, we also offer Care Credit (www.carecredit.com), an interest free financial option for qualifying patients.   Due to the number of insurance plans, we suggest you contact your insurance company if you have specific questions regarding your coverage. 

After surgery, we will file your insurance papers for you.  If your responsibility is more than anticipated, you will be billed for the difference.  Any overpayment will be refunded to you. 

It is your right as a patient to request information or have questions answered regarding our fee or payment policies.  If you have any questions or problems regarding your bill, please contact our business office.  We will be happy to help you between the hours of 8:30 AM and 5:00 PM, Monday through Friday at 502-897-7401.

 

Cosmetic Surgery Fees:

Facility and anesthesia fees for cosmetic procedures will be collected on or before the day of surgery.
